Community Needs Assessment
T.J. Samson Community Hospital continues to research the most effective ways to best serve the needs of the community in which it serves. Over the past three years, the organization has participated in conducting a Community Needs Assessment in partnership with the Barren River Community Health Planning Council. The Community Health Needs Assessment determined the areas of greatest health need for the region. Below you will find a copy of the Community Health Needs Assessment and the Executive Summary published by the Barren River Community Health Planning Council. Also, shown below is T.J. Samson Community Hospital's Community Health Needs Assessment and Strategic Implementation Plan, that demonstrates how the hospital plans to help meet the needs identified within the community assessment. The conducting of the Community Health Needs Assessment and the implementation of the Strategic Plan are both in an effort to comply with the Affordable Care Act of 2010 and to maintain organizational nonprofit status.
