Health Information Management Department
(Medical Records)
The health information management department (HIM) (medical records) is a support service of T.J. Samson Community Hospital. Its primary purpose is to contribute to the quality of patient care through the development and maintenance of a comprehensive, centralized medical records system. The HIM department ensures that the hospital has accurate and complete medical records for each patient admitted and uses required guidelines to ensure that medical records are accurate, timely, complete, accessible and secure.
The HIM department is available 24 hours a day, seven days a week. If you have questions regarding patient records, or to obtain medical records, contact the HIM department at (270) 651-4447. If you need verification of birth, please call (270) 651-1873.
