Home Care Services
Services provided by T.J. Samson Community Hospital Home Care Program include home health, hospice and home medical equipment.
Home Health
Home Health includes a wide variety of services such as skilled nursing, psychiatric nursing, home health aides, physical therapy, occupational therapy, speech therapy and clinical respiratory therapy. Additional programs include:
- Medicaid Home and Community-based Waiver: Offers personal care, homemaking and respite services.
- First Steps: Offers specialized care for infants and toddlers with disabilities or developmental delays.
- Early Periodic Screening Diagnosis and Treatment: A Medicaid program for children ages 3 to 18 with disabilities or developmental delays.
Services are provided in Barren, Hart and Metcalfe counties.
Hospice
Hospice is a program provided for patients who have been diagnosed with a terminal illness and have a life expectancy of six months or less. Hospice consists of an interdisciplinary team that includes physician services, nursing services, social services (counseling, home care aides and homemaker services), spiritual services, volunteer services, bereavement services, and specialized services when necessary.
Hospice is a special concept of care designed to provide comfort and support to patients and their families. Hospice services are provided in the Barren River Area Development District (BRADD).
Home Medical Equipment
T.J. Samson home medical equipment provides durable medical equipment, such as home oxygen, which includes liquid oxygen as well as concentrator and gaseous oxygen. Also available are continuous positive airway pressure machines (CPAPs), bilevel positive airway pressure machines (BiPAPs) and home ventilators. Disposable medical supplies are also provided as ordered by physician.
The T.J. Samson Community Hospital Home Care Program is located on the campus of T.J. Samson Community Hospital and is open Monday through Friday from 8 a.m. to 4:30 p.m. Services are available 24 hours per day, seven days per week with the on-call system.
